Transitional Services

Transitional Services includes our 20-bed Skilled Nursing Facility (SNF) and our 19-bed Hendrick Center for Extended Care, which is a Long Term Acute Care Hospital (LTACH). Both units are for patients who are transitioning from the acute care hospital to another level of care.
